I agree to abide by the general rules of conduct prescribed for participation in THE FORGE. I understand that among these rules are prohibitions against abusing officials, repeated profanity, flagrant fouls, throwing equipment and any other unsportsmanlike behavior. I further understand that violations against these rules will result in game/bootcamp ejections, suspensions, and/or possible permanent dismissal from the league. Wavier and Release of Lability I understand that participating in THE FORGE and events organized by THE FORGE is a potentially dangerous and hazardous activity that includes risks, including - but not limited to, the possibility of physical harm or injury, permanent disability, death and damage to property. I understand that I am liable for my own safety, maintaining health and will not hold THE FORGE liable should I become injured or ill. COVID-19 is a novel illness and con be transmitted through physical contact or airborne pathways. I understand the risks inherent with physical activity and proximity to others during activity. I will not hold THE FORGE liable or responsible should I contact COVID-19 or any other illness. Having read and signed this Waiver and Release of Liability I am knowing, understanding and willing to assume the risks associated with the participation in THE FORGE activities. I understand that THE FORGE does not assume any responsibility or obligation to provide financial assistance or other assistance, including but not limited to medical, health or disability insurance, in the event or injury or illness to myself caused in any part by my participation in THE FORGE activities. I hereby release and further discharge THE FORGE from any claim whatsoever that arises or may arise by my participation.
